All students entering a public school for the first time and students entering Grade 9 must provide documentation of a health examination within thirty days after entry. The examination must be current (completed within two years of entry) and conducted by a currently licensed physician, advanced practice nurse, nurse practitioner, or physician’s assistant. Acceptable forms are:

• Health Examinations and Screening (Title 14, Regulation 815)
• Student Health Forms
  • PreK-Grade 6 Health Form
  • Grades 7-12 Health Form

Lead Testing
Regulation 815: Health Examinations and Screening states that children who enter school at kindergarten or at age 5 or prior, shall be required to prove lead screening per 16 Del.C. Ch.26 (Childhood Lead Poisoning Prevention Act).

Tuberculosis Screening
Delaware requires tuberculosis (TB) screening for all students entering a public school. School staff and volunteers are assessed routinely for exposure to TB or active disease. Anyone with a positive screening is required to have a follow-up medical assessment.

Hand Sanitizer
If your child is unable to use hand sanitizer for any reason, please contact your school nurse. If your student may need assistance applying hand sanitizer properly, please fill in this portion of this form and give the completed slip to your student’s school nurse.

Parents/Guardians of students enrolled in the Christina School District must complete an Online Acknowledgment Form for the 2025-2026 school year to acknowledge that you have been made aware of the following forms and agree to complete and return them to the school, as necessary. Please visit your child's school website for "Back to School" forms.

Individuals can order in person at any of three Office of Vital Statistics locations around the state. All locations are open for customer service Mondays to Fridays from 8:00 a.m. to 4:00 p.m. except on holidays.
