Information for Parents
New students entering McVey must provide:
- An up-to-date immunization record
- A copy of a physical exam done within the past 12 months
- A TB skin test or TB risk assessment done within the past 12 months
In addition, a Kindergarten student must show documentation of a lead screening done some time in his/her life.
Medications
All medications brought into school must be in the original container with a prescription label in place, (if prescribed), in order for the school nurse to administer medications to students during the school day. In addition, a written note from the parent/guardian is also required. Without all this information the school nurse is unable to administer medication to the student in school.Please note: All medications (controlled substance medications, non-controlled prescription medications and over-the-counter medications), must be picked up by a parent/guardian by the last day of school, and/or, if the child transfers or withdraws to another school. Medications for your child which remain in the school nurse's office will be disposed of on the last day of school.
If a parent/guardian anticipates the his/her child will require medication while on a field trip, a contract must be signed between the parent/guardian and the child's teacher regarding administration for the medication. The medication must be sent separately from what the nurse has available in the nurse's office.
