The Christina School District will be offering the COVID-19 vaccine for students 3 years of age and older. There is no cost for the vaccine or charge to your insurance. Students aged 3-17 must have parental consent. Students age 18 and older may complete the forms themselves.
Families with unvaccinated students aged 3 or older are encouraged to participate in this opportunity. Our medical partners will be offering the Pfizer vaccine for students aged 3 and older. Preregistration is required.

- Make an appointment for each child receiving the vaccination.
- Download and print the completed form from the registration page, or print and complete a consent form for each student.
- Bring the completed consent form to the vaccination clinic.
- Bring an insurance card for each child, if you have one. It is not required.
- Please dress your child in clothing that has easy access to their arms.

All students who receive the vaccination will be provided a vaccination card. Please keep this card in a safe place as it will be needed for the second vaccination and booster.
Parent Consent Form
Signed Parental Consent Forms (Informed Consent for Immunization with COVID-19 Vaccine) will be required prior to your child being vaccinated. The Informed Consent for Immunization with COVID-19 Vaccine form can be found below, please have the form printed, completed and set with your child on the date of vaccination. Print and bring this form to the vaccination clinic.
