COVID-19 Vaccinations

  • COVID-19 Vaccinations for students age 12+

    The Christina School District will be offering the COVID-19 vaccine for students 12 years of age and older. There is no cost for the vaccine or charge to your insurance. Students aged 12-17 must have parental consent. Students age 18 and older may complete the forms themselves. 

    Families with unvaccinated students aged 12 or older are encouraged to participate in this opportunity. Our medical partners will be offering the first dose of the Pfizer vaccine on October 26 for our middle school students and on October 27 for our middle school students at Bancroft and Bayard and high school students.

    Middle School Vaccination Events

    • Gauger-Cobbs, Kirk & Shue-Medill - The vaccination clinic for middle school students will be held at Kirk Middle School from 9:00-11:00 AM on October 26. We will provide transportation to and from school to the vaccination clinic at Kirk Middle School for students who have completed and returned the UNIVERSAL VACCINE CONSENT FORM. Students will return to their home school to finish the school day. 

    • Bancroft (grade 6-8) & Bayard (Pyle - grades 7 & 8) -The vaccination clinic for middle school students will be held at The Bancroft School from 9:00-11:00 AM on October 27 for students who have completed and returned the LIFE HEALTH CENTER VACCINATION FORMStudents at Bayard (Pyle) will walk with adult escorts to the Bancroft School at the designated time. Students will return to their home school to finish the school day. 

    • Bayard (Pulaski - grade 6) -The vaccination clinic for middle school students will be held at The Bayard School (Pulaski Campus) from 9:00-11:00 AM on October 27 for students who have completed and returned the LIFE HEALTH CENTER VACCINATION FORM

     

    High School Vaccination Event

    The vaccination clinic for high school students will be held at Christiana High School from 9:00-11:00 AM on October 27. We will provide transportation to and from school to the vaccination clinic at Christiana High Schools for students who have completed and returned the UNIVERSAL VACCINE CONSENT FORM. Students will return to their home school to finish the school day.   

     

    Vaccination Cards

    All students who receive the vaccination will be provided a vaccination card. Please keep this card in a safe place as it will be needed for the second vaccination scheduled for November 16 (middle schools) and November 17 (Bancroft, Bayard and high schools). We will utilize the same procedure for the second dose clinics. Your school nurse will also communicate that appointment time at a later date.

    Parent Consent Form

    Signed Parental Consent Forms (Informed Consent for Immunization with COVID-19 Vaccine) will be required prior to your child being vaccinated. The Informed Consent for Immunization with COVID-19 Vaccine form can be found below, please have the form printed, completed and set with your child on the date of vaccination.
